Materials:

stock beveled glass pieces
pistol-grip cutter
pattern
safety glasses
breaking pliers
copper foil
burnishing tool
flux
brush
soldering iron
solder
soap and water
metal strip
duct tape
clear silicone caulking

Steps:
  1. Place the pieces of glass over the pattern, and cut each out with a glass cutter, scoring the glass and then carefully breaking it apart. For pieces that are too small to break with your fingers, use a pair of breaking pliers.
  2. Center a strip of copper foil on the edge of each piece of glass, and wrap the foil around all the edges. Burnish the foil all over.
  3. While working in a well-ventilated area, paint flux (cleaner) over all the copper areas.
  4. Working on a flame-retardant surface, solder all the foil areas. The solder will adhere only to the metal and not to the glass.
  5. Turn the piece over when it's all soldered on one side, and repeat the process on the other.
  6. When the soldering is complete, wash the piece with soap and water.
  7. Cut the metal strip to fit the sides of the panel, fit the glass into the track in the strip, and solder it into place.
  8. To mount the panel in a window, "piggyback" it over an existing glass pane. Fit it into place and tape it with duct tape on two sides
  9. Apply a line of clear caulking around the other two sides. Let it dry overnight.
  10. Remove the tape and caulk the remaining sides.
